Alteration/Modification Request Form
An Alteration/Modification Request must be submitted to Casa Bella Property Management and approved by the Board of Directors prior to making any alterations or modifications to your unit's

All requests can be submitted using one of two methods. You can either
1) print out this blank Alteration/Modification Request Form and then complete the necessary information and either mail or fax it to Casa Bella (Casa Bella Property Management, 850 N. Crooks, Suite 100 Clawson, MI 48017; Phone: Office: 248-655-1500 or Fax: 248-655-3900). They will then scan and electronically forward your request to the members of the Board of Directors for their review and approval.
2) login to your account on the Casa Bella web site and select "Service Requests" from the menu on the left; then click on the magnifying glass icon next to "Modification Request". You can then fill out the on-line form, attach a digital picture or scan of any supporting documentation and then at the bottom of the form, type your signature and then click on "Submit". The Board of Directors can read and review your request as soon as it has been submitted. The approval process is usually quicker with this method as compared to the first method.
Please note that regardless of the method used, the approval process can sometimes take several days, or even weeks, to complete and that you cannot proceed until you have received the Board's approval.
